NHS CPFT Adult Locality Team – Fenland

Fenland Adult Locality Team (FALT) is for those over 17 living and registered with a Fenland GP who are experiencing symptoms of moderate to severe mental illness. The team assess, provide treatment and signpost to relevant services and support people in the community.

What to Expect:

Once you have been referred you will be contacted by the team in order to arrange a face to face assessment appointment, usually at Agenoria House.

At the assessment you will be asked about your current mental health, your mental health history, what treatment you have had in the past and what worked, and what your current goals are around your mental health. If you are feeling anxious and would like a carer, family member or loved one to attend with you, they can sit in the appointment or wait in the waiting room.

Following the assessment you will be allocated a care coordinator, there may be a wait for this however you will be offered a 6 week course whilst you wait for support. This course aims to support with your mental health whilst you wait and prepare you for treatment, there is also a duty worker available each day to provide telephone support and arrange a face to face appointment if needed.

Once you are allocated a care coordinator they will work with you on a variety of treatments and therapies to support you in reaching your recovery goals. The service is holistic and will ask if your family members or carers need any support, they can refer on to carers’ support if needed.

THIS SERVICE IS BY PROFESSIONAL REFERRAL ONLY. Referrals are accepted from the Primary Care Mental Health Service, FRS, Crisis team, Inpatient wards, Liaison Psychiatry, other MH teams, LADs and GPs. The locality team support those experiencing symptoms of mental illness which may include:

  • Bipolar Illness
  • Depression
  • Personality Disorder
  • Anxiety Disorder
  • Obsessive Compulsive Disorder (OCD)
  • Post-Traumatic Stress Disorder (PTSD)
  • Psychosis / Schizophrenia
